Need LAMP developer for web form creation

I humbly apologize for posting to this forum without more knowledge. I did look for the faq and was unable to find it. If this is not the correct forum, please let me know. There is an ambulance company in Houston that has seen an increase in business more than 10x since Katrina. They were already working on a way to move away from paper forms but the developer is a month behind and unresponsive. They are unable to transport as many patients currently because the documentation is a manual process and is very slow. I can explain more if interested. The working form is at http://service.dfmco.com/aemstar/aemsdemo10h9.html. The problem we are having is that we need a way to list current records in the database in reverse chronological order and to update records already written to the database. I can provide the current code and access to the server. There is also a send form that is needed that is very similar to the first and I am hoping that the cuurrent form can just be copied and modified. The drop dead date is Monday morning.
